PROGRAM GUIDELINES

After you purchase five clubs from an authorized U.S. Kids Golf retailer or on our website, you're eligible to receive a 6th Club Free. Follow these simple steps below:

  • Purchase must be made from an authorized U.S. Kids Golf Retailer.
  • Used or Dullet Golf Clubs are not eligible for the 6th Club Free Program.
  • Collect UPCs for at least 5 clubs. (The UPC is the bar code-sticker located on the shaft of individual clubs or on the art box for full sets.)
  • Original register receipt(s) or U.S. Kids Golf Packing Slip(s).

CLUB COMBINATIONS

  • Clubs are totaled individually, but must equal five (5) clubs.
  • 3-club and 4-club sets require additional purchase to equal five (5) clubs.
  • For 7-club sets, retain two (2) UPCs for your next redemption opportunity!

COMPLETE THE FORM AND BE SURE TO STAPLE THE FOLLOWING ITEMS TO THE FORM:

  • The original UPC(s)
  • Cash register receipt(s) or U.S. Kids Golf Packing Slip(s).

Please allow up to two weeks from U.S. Kids Golf receipt to receive your redemption code via email. The code is valid for one time use and you will have 30 days to redeem it in our online store. Regular shipping rates apply. Faxed copies are not accepted.
