6th Club Free - US Details of the Program

Steps for the program:

1. Collect UPC codes and purchase receipts.  The UPC code is the bar code located on the box of a set or on the hang card of the individual clubs.  Starting in 2007 product has removable UPC stickers on the shaft of the clubs.
2. Login or join the USKG Club (membership is free).
3. Download the 6th Club Free form that you need.  If you purchased Ultralight clubs only Ultralight clubs may be redeemed.  If you purchased TOUR SERIES clubs you may redeem either Ultralight or TOUR SERIES clubs.  Golf Balls are also available.  Please see the forms for available items.
4. Select the club you would like to receive.  Only items on the form are available.
5. Include a check or credit card number for the $8 shipping and handling charge per club ($11 per club for residents of Hawaii or Alaska).
6. Mail all required items to U.S. Kids Golf.
7. Allow 3-4 weeks for delivery.

Exclusions:
  - This program is available to United States residents only (excluding Puerto Rico and the US Virgin Islands).
  - If Ultralight clubs are purchased only Ultralight clubs may be redeemed.
  - If TOUR SERIES clubs are purchased TOUR SERIES or Ultralight my be redeemed. 
  - Offer excludes retailers.
  - U.S. Kids Golf reserves the right to refuse an order.
  - U.S. Kids Golf reserves the right to change or cancel this program at any time without notice. 
  - Purchases from Ebay or other 3rd party entities do not qualify. 
  - Faxed or email orders will not be accepted.
  - When you are ready to redeem your free club, login to the USKG Club
